Forensic Housing Case Manager

Summary of Position Function
To Provide housing case management services and support to adult individuals being released from Correctional Facilities directly into the Community and/or from New York State Psychiatric Centers into the community. These individuals are identified as having serious and persistent mental health issues and are often linked with NYS Parole.
Education Requirements
- Bachelor’s Degree in Human Services or related field plus one year work experience with mental health recipients OR
- Associate degree with two years’ experience working with mental health recipients or high school diploma with four years’ experience working with mental health recipients
- High School Diploma with four years working with mental health recipient
Experience
- One – four years work-related experience dependent on education
Must possess valid Driver’s license and an acceptable driving record.

Major Duties & Responsibilities
- Knowledge and demonstration of agency core values in day-to-day activities
- Assist individuals in locating safe and affordable housing in an area of their choice within Monroe County
- Purchase with the individual household furnishings for their home
- Assist with obtaining benefits if needed, collect income, diagnostic and complete a rental stipend to determine voucher per month
- Maintain open communication with all treatment and service providers, families and any other supports or networks the individual is working within the community.
- Assist in outreach and location of the individuals referred
- Responsible for recordkeeping (progress notes) in the Cerner Database System and other required paperwork such as service planning, roster maintenance, rental stipends, income and homeless verifications, and any other duties asked by supervisor of supportive housing
- Must have own transportation as there will be a minimum of two visits per month with assigned clients, preferably in their home(s)
- Attend team and community-based team meetings in the community
SKILLS:
- Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc.
- Knowledge of Community Health Systems.
